Registration Information

Please be advised that your registration name must match your identification. (Passport) .NOTE: A passport is required for this cruise. Once you have registered, name changes are NOT allowed. Any date or time changes to your itinerary are subject to revision fees plus the difference in fare which you will be responsible for.

Registration can be completed online.  To register online please follow the registration procedures below.

• On-line Registration Procedures:  To register for this program click the Register for the First Time link below.  The online registration system will open in a new window.
   
• Guest Information:  Please complete the name, address and phone information for the first traveler and then click Add Additional Travelers to input information for your guest. Please use your legal name.  Your passport must match the name on your reservation.
   
• Confirmation:  A summary screen of your registration will be displayed.  Click Print Registration to print your confirmation.
     
• To Make Changes:  Contact Suzi at Columbus Groups+Incentives with any changes to your registration information suzi@columbusincentives.com
   
• To Access Your Record:  Click Access Modify Trip below and enter the password you entered when you registered.
   
• By completing a registration you agree to the following TERMS AND CONDITIONS.

Deviation from program dates:

• A fee of $50 per person plus any additional fees assessed by the airline, hotel, ground transfer company, or other travel suppliers involved, will be charged for any deviation from the group travel dates shown on the itinerary page. Dental Select is not responsible for these fees. You will need to provide payment to Columbus Travel.
